Player Registration Form
If your child is interested in trying out for one of our traveling teams the first step is to complete our Registration Form. Please enter the players and parents information along with phone numbers. Get Player Registration form.
Player Code of Ethics Form
Players must read the Players Code of Ethics before they can participate in our program. The bottom of the form must be signed and dated. Get Players Code of Ethics form.
Player Liability Waiver Form
If your child has been selected to participate on one the traveling team you as the parent must complete the Player Liability Waiver Form. This form releases the Jordan Basketball Association from being liable for any injuries that may occur to your player while attending a game or tournament. Get Player Liability form.
Player Uniform Agreement
After completing the Player Liability Waiver form each parent must complete the Player Uniform Agreement. This form gives the association permission to issue each player a uniform that must be returned at the end of the season. Get Player Uniform Agreement.
Parent Code of Ethics Form
Parents of players must read the Parents Code of Ethics before their child can participate in our program. The bottom of the form must be signed and dated. The form must be then given to the childs coach who will submit it to the association staff.

Coaches Code of Ethics Form
All coaches in our association must read the Coaches Code of Ethics before they can coach a team in our program. The bottom of the form must be signed and dated, then turned into our association staff. Get Coaches Code of Ethics form.
